# PATIENT RESOURCE CENTER

## Patient Forms

It is helpful for our office to receive your patient information prior to your appointment. Patients will generally receive a text link to complete electronic forms prior to their appointment. You also have the option of downloading forms from our website and completing them manually, which requires a PDF reader (like Adobe Acrobat). You may email completed forms to scheduling@entlubbock.com, fax them to 806.788.0470 or simply bring the completed forms to your appointment. If you prefer to fill them out in our office, we recommend you arrive 15-20 minutes early for your appointment.

 

Note that there will be a **$35 fee for appointments missed or cancelled with less than 24 hours notice**. Our office closes at noon on Friday, so Monday afternoon appointments need to be confirmed before then to avoid a cancellation fee. Please be considerate when canceling or rescheduling an appointment so that we may have enough time to accommodate other patients on our wait list. Thank you for your understanding.

## Patient Information

Below is information pertaining to some of our most popular procedures. More in-depth information may be available on our blog page for these and many other procedures. Please follow the instructions for preparation relating to your specific procedure. If you have any questions relating to your procedure that are not answered in the corresponding documents below or on our blog, please contact us!

 

There will be a **$250 fee for surgeries missed or cancelled with less than 3 business days notice.** You are responsible for notifying our office directly, NOT the surgery center or facility, if you wish to cancel for any reason. Please be considerate when canceling or rescheduling a surgery or appointment so that we may have enough time to accommodate other patients.
