Patient Resource Center
It is helpful for our office to receive your patient information prior to your appointment. You will need a PDF reader (like Adobe Acrobat), to download and fill out our forms on your electronic device or computer. You may also print the forms and fill out by hand. You may email completed forms to scheduling@entlubbock.com, fax them to 806.788.0470 or simply bring the completed forms to your appointment. If you prefer to fill them out in our office, we recommend you arrive 15-20 minutes early for your appointment.
Note that there will be a $35 fee for appointments missed or cancelled with less than 24 hours notice. Please be considerate when canceling or rescheduling an appointment so that we may have enough time to accommodate other patients on our wait list. Thank you for your understanding.

Financing
Your medical, surgical and hearing care needs can be financed! Our office is proud to offer healthcare financing through Care Credit. Care Credit plans are developed to assist both the patient and physician, and include no interest and extended period payment plans.
